ICADA 2015 Richtlijnen

  1. Vlooien uitsluiten
    • Controleer op vlooien en vlooienontlasting (visuele controle en kammen) en  behandel tegen vlooien indien aangetoond
    • Start en/of ga verder met vlooienpreventie, allergische patiënten kunnen jeukklachten ontwikkelen zonder dat er vlooien gedetecteerd worden
  2. Andere ectoparasieten uitsluiten 
    • Visuele controle en kammen/borstelen 
    • Schurft (Sarcoptes) behandelen, indien aanwezig 
    • (Meerdere) diepe huidafkrabsels voor detectie van Demodex spp. 
    • Start en/of ga verder met behandeling en preventie van ectoparasieten
  3. Sluit (overgroei) pyodermie uit 
    • Visuele inspectie van de huid op pustels, exsudaat, etc. 
    • Cytologie om gisten en bacteriën aan te tonen of uit te sluiten 
    • Topicale behandeling met chloorhexidine bij positieve cytologie 
    • Orale antimicrobiële therapie indien chloorhexidine niet voldoende is
  4. Voedselallergie uitsluiten 
    • Eliminatiedieet pas opstarten nadat stap 1-3 volledig zijn doorlopen
    • Dieet minimaal 8 weken volhouden 
    • Bij onvoldoende resultaat: 
      • Onderzoeken of dieet strikt werd gevolgd
      • Andere dieetsamenstelling overwegen 
    • BELANGRIJK: rechallenge: na verbetering van de klachten opnieuw het oude voer geven 
      • Klachten blijven weg => geen voedselallergie 
      • Pruritis komt terug => voedselallergie bevestigd
  5. Immuuntherapie 
    • Immuuntherapie pas opstarten als stap 1-4 volledig zijn doorlopen.
    • Gebruik de criteria van Favrot*
    • Bepaal de oorzakelijke agentia met serologie en/of huidtesten 
    • Start immuuntherapie voor bevestiging van de diagnose (en pas na succesvolle behandeling van de ernstigste pruritis)
    • Zet immuuntherapie voort indien goed resultaat

*Criteria van Favrot voor canine atopische dermatitis

  1. Eerste symptomen <3 jaar leeftijd
  2. Vooral bij honden die binnenshuis verblijven
  3. Glucocorticoïde responsieve pruritis
  4. In eerste instantie pruritis zonder huidbeschadiging
  5. Voorvoeten aangetast
  6. Oorschelpen aangetast
  7. Randen van de oorschelp niet aangetast
  8. Dorsolumbale gebied is niet aangetast
Voldoet een patiënt aan 5 criteria, dan ligt de sensitiviteit op 85% en de specificiteit  op 79% om een patiënt met CAD te differentiëren van een patiënt met chronische terugkerende pruritis zonder CAD. Een zesde criterium verhoogt de specificiteit tot  89%, maar verlaagt de sensitiviteit naar 58%.
